Se calcula que en tiempos de la tracción animal, el 25% de la superficie cultivada era necesaria para mantener a estas “máquinas prehistóricas”. Actualmente nos encontramos frente a un nuevo paradigma técnico y socioeconómico en la agricultura, con cada vez más grandes y “voraces” herramientas en nuestros campos: las últimas generaciones de tractores de extrema potencia. ¿Cuál sería la superficie equivalente para satisfacer sus necesidades de energía en caso del uso de biocombustibles procedentes de cultivos alimentarios? ¿Está el modelo productivo actual adaptado a esta “sobrecarga”? ¿Son los tractores un ejemplo actual de involución tecnológica? Enfoque de intercambio social y la felicidad en el trabajo: explorando el efecto mediador del compromiso organizacional
This article aims to determine the direct and indirect effects of social exchange relationships Leader Member Exchange and Team Member Exchange (LMX and TMX) on organizational commitment and happiness at work. Based on the literature review and relying on the Social Exchange Theory perspective, the authors constructed the hypothetical structure, including the four variables under study. The sample comprised 177 employees from Mexican firms, and the proposed hypotheses were under examination through a path analysis. Results show that LMX, TMX, and happiness at work have a direct effect on organizational commitment. We also found an organizational commitment to be a partial relationship mediator between LMX and happiness at work.Este artículo tiene como objetivo determinar los efectos directos e indirectos de las relaciones de intercambio social, intercambio líder miembro e intercambio entre miembros del equipo (LMX y TMX) sobre el compromiso organizacional y la felicidad en el trabajo. Con base en la revisión de la literatura y apoyándose en la perspectiva de la Teoría del Intercambio Social, los autores construyeron la estructura hipotética, incluyendo las cuatro variables en estudio. La muestra estuvo compuesta por 177 empleados de empresas mexicanas, y las hipótesis propuestas fueron examinadas a través de un análisis de trayectoria. Los resultados muestran que LMX, TMX y la felicidad en el trabajo tienen un efecto directo sobre el compromiso organizacional. También encontramos que el compromiso organizacional media la relación parcial entre LMX y la felicidad en el trabajo
Finding the best available techniques for an environmental sustainable waste management in the fish canned industry
This work proposes the use of the life cycle assessment (LCA) to identify the best available techniques (BAT) to the management of the residues generated in the anchovy canning sector. This industry generates huge amount of solid and liquid wastes, and their management is one of the hotspots of the canned anchovy life cycle. The application of BATs can improve the environmental performance of the canned anchovy. However, sometimes it is not clear which BAT is the most appropriated, and an environmental analysis is required. In this sense, several BATs are proposed based on the circular economy concept, which promotes the reutilisation of wastes and they were evaluated under a life cycle approach: (i) valorisation of the anchovy residues into fishmeal, fish oil and anchovy paste, (ii) incineration and (iii) disposal in a municipal solid waste landfill. The LCA was conducted from cradle to gate using the global warming (GW) indicator. The burden of hospital malnutrition in Spain: methods and development of the PREDyCES® study
It is well known that hospital malnutrition is a
highly prevalent condition associated to increase
morbidity and mortality as well as related healthcare
costs. Although previous studies have already
measured the prevalence and/or costs of hospital
nutrition in our country, their local focus (at regional
or even hospital level) make that the true prevalence
and economic impact of hospital malnutrition
for the National Health System remain unknown in
Spain. The PREDyCES® (Prevalence of hospital
malnutrition and associated costs in Spain) study
was aimed to assess the prevalence of hospital malnutrition
in Spain and to estimate related costs.
Some aspects made this study unique: a) It was the
first study in a representative sample of hospitals of
Spain; b) different measures to assess hospital malnutrition
(NRS2002, MNA as well as anthropometric
and biochemical markers) where used both at
admission and discharge and, c) the economic consequences
of malnutrition where estimated using the
perspective of the Spanish National Health System.Es bien sabido que la desnutrición hospitalaria es un proceso
altamente prevalente asociado al aumento de la morbilidad
y mortalidad, así como a elevados costes sanitarios. Aunque
estudios previos han medido la prevalencia y/o los costes
de la nutrición hospitalaria en nuestro país, su enfoque local
(regional o incluso a nivel hospitalario) hacen que la verdadera
prevalencia e impacto económico de la desnutrición
hospitalaria para el Sistema Nacional de Salud sean aún desconocidos
en España. El objetivo del estudio PREDyCES®
(Prevalencia de la Desnutrición hospitalaria y los Costes asociados
en ESpaña) fue evaluar la prevalencia de la desnutrición
hospitalaria en España y estimar sus costes asociados.
Algunos aspectos de este estudio lo hicieron singular: a) Fue
el primer estudio de este tipo con una muestra representativa
de los hospitales de España, b) se utilizaron diferente medidas
para evaluar la desnutrición hospitalaria (NRS 2002,
MNA, así como marcadores antropométricos y bioquímicos)
tanto en el momento del ingreso como al alta hospitalaria y, c)
se estimaron las consecuencias económicas de la desnutrición
desde la perspectiva del Sistema Nacional de Salud español
Use of radon and CO2 for the identification and analysis of short-term fluctuations in the ventilation of the polychrome room inside the Altamira Cave
A study is presented on rapid episodes of air exchange in the Polychrome Room of the Altamira Cave (Cantabria, Spain) using continuous monitoring of radon and CO2 tracer gases, as well
as environmental parameters such as internal and external air temperature. For this, criteria have been developed to carry out an inventory of these types of events during the 2015–2020 period. Most of the degassing-recharging events occur over several hours or days, especially during spring and autumn.
This means that the room can be significantly ventilated during these short periods of time, posing an exchange of energy and matter with potential impact in the preservation of the rock art present inside.
In addition, the hypothesis that temperature gradients between the internal and external atmosphere is one of the main factors that induces degassing has been tested.This research was funded by the Spanish Ministry of Culture and Sport, grant number J200028
Prevalence and costs of multinutrution in hospitalized patients; the PREDyCES study
Justificación y objetivos: El estudio PREDyCES® tuvo dos
objetivos principales. Primero, analizar la prevalencia de
desnutrición hospitalaria (DH) en España tanto al ingreso
como al alta, y segundo, estimar sus costes asociados.
Métodos: Estudio nacional, transversal, observacional,
multicéntrico, en condiciones de práctica clínica habitual
que evaluó la presencia de desnutrición hospitalaria al
ingreso y al alta mediante el NRS-2002®. Una extensión
del estudio analizó la incidencia de complicaciones asociadas
a la desnutrición, el exceso de estancia hospitalaria y
los costes sanitarios asociados a la DH.
Resultados: La prevalencia de desnutrición observada
según el NRS-2002® fue del 23.7%. El análisis multivariante
mostró que la edad, el género, la presencia de
enfermedad oncológica, diabetes mellitus, disfagia y la
polimedicación fueron los factores principales que se asociaron
a la presencia de desnutrición. La DH se asoció a
un incremento de la estancia hospitalaria, especialmente
en aquellos pacientes que ingresaron sin desnutrición y
que presentaron desnutrición al alta (15.2 vs 8.0 días; p <
0.001), con un coste adicional asociado de 5.829€ por
paciente.
Conclusiones: Uno de cada cuatro pacientes en los hospitales
españoles se encuentra desnutrido. Esta condición
se asocia a un exceso de estancia hospitalaria y costes asociados,
especialmente en pacientes que se desnutren
durante su hospitalización. Se debería generalizar el cribado
nutricional sistemático con el objetivo de implementar
intervenciones nutricionales de conocida eficacia.Background and aims: The main objective of the
PREDyCES® study was twofold. First, to analyse the
prevalence of hospital malnutrition in Spain, both at
admission and at discharge, and second, to estimate the
hospital costs associated with disease-related malnutrition.
Methods: The study was a nationwide, cross-sectional,
observational, multicentre study in routine clinical practice,
which assessed the prevalence of hospital malnutrition
both at patient admission and discharge using NRS-
2002®. A study extension analysed the incidence of
complications associated with malnutrition, excess
hospital stay and healthcare costs associated with hospital
malnutrition.
Results: Malnutrition was observed in 23.7% of
patients according to NRS-2002®. Multivariate analysis
revealed that age, gender, presence of malignant disease,
diabetes mellitus, dysphagia and polymedication were the
main factors associated with the presence of malnutrition.
Malnutrition was associated with an increase in
length of hospital stay, especially in patients admitted
without malnutrition but who presented malnutrition at
discharge (15.2 vs. 8.0 days, p < 0.001), with an associated
additional cost of €5,829 per patient.
Conclusion: In Spanish hospitals, almost one in four
patients is malnourished. This condition is associated
with increased length of hospital stay and associated
costs, especially in patients developing malnutrition
during hospitalization. Systematic screening for malnutrition
should be generalised in order to implement nutritional
interventions with well-known effectiveness
The Spanish society of Parenteral and Enteral Nutrition (SENPE) and its relation with healthcare authorities
Está muy bien documentado en la literatura médica
que la desnutrición es un problema común en todos los
niveles de atención sanitaria, desde atención primaria a
especializada y en centros de atención geriátrica. Este
problema no se limita a países con pocos recursos económicos
o con limitado desarrollo social y económico. También
es un problema universal en Europa. La desnutrición
aumenta las cifras de morbilidad, mortalidad,
ingresos hospitalarios y duración de la estancia. Estas
cifras más elevadas suponen lógicamente un aumento del
uso de recursos sanitarios. A pesar de esto, el problema de
la desnutrición a menudo puede pasar desapercibido y el
paciente no recibir el tratamiento necesario. Este problema
requiere la cooperación de múltiples agentes tales
como los Gobiernos de los Estados, los profesionales de la
salud y los mismos ciudadanos. El VIII Foro de Debate
concluye con la necesidad de establecer un claro plan de
actuación (a semejanza de la European Alliance for
Health Nutrition) y la creación de una plataforma (coalición)
que reúna las voces de asociaciones de profesionales
sanitarios, instituciones, colegios profesionales, asociaciones
de pacientes, industria y entidades aseguradoras. Los
fines de esta plataforma consistirán en informar de la
extensión del problema, identificar y potenciar líderes
que transmitan los fines de esta iniciativa ante las autoridades
autonómicas y nacionales, propuesta de soluciones
y colaboración en su puesta en marcha y finalmente, evaluación/
control de las acciones desarrolladasIt has been well documented in medical literature that
hyponutrition is a common issue at all healthcare levels,
from primary to specialized health care, as well as geria -
tric healthcare facilities. This problem is not limited to
countries with scarce economic resources or limited social
development; it is also a universal issue in Europe.
Hyponutrition increases the rates of morbidity, mortality,
hospital admissions, and hospital stay. These higher
figures also represent a higher use of healthcare
resources. In spite of this, hyponutrition may often go
undetected and the patient may not receive the necessary
treatment. This problem requires the cooperation of multiple
agents such as the Governments, the healthcare professionals,
and the citizens themselves. The VIII Discussion
Forum concludes on the need to establish a clear-cut
plant for action (similar to the European Alliance for
Health Nutrition) and the creation of a platform (coalition)
encompassing the voices of healthcare professionals
associations, institutions, professional colleges, patients
associations, the pharmaceutical companies, and insurance
companies. The goals of this platform will be to
inform about the extent of this issue, to identity and promote
leaders that will convey the aims of this initiative to
regional and national healthcare authorities, to present
solutions and to collaborate in their implementation, and
finally to assess/control the actions take
Nutrición enteral domiciliaria en España: registro Nadya del año 2011-2012
Objective: To describe the results of the home enteral
nutrition (HEN) registry of the NADYA-SENPE group in
2011 and 2012.
Material and methods: We retrieved the data of the patients
recorded from January 1st 2011 to December 31st 2012.
Results: There were 3021 patients in the registry
during the period from 29 hospitals, which gives 65.39 per
million inhabitants. 97.95% were adults, 51.4% male.
Mean age was 67.64 ± 19.1, median age was 72 years for
adults and 7 months for children. Median duration with
HEN was 351 days and for 97.5% was their first event
with HEN. Most patients had HEN because of neurological
disease (57.8%). Access route was nasogastric tube
for 43.5% and gastrostomy for 33.5%. Most patients had
limited activity level and, concerning autonomy, 54.8%
needed total help. Nutritional formula was supplied from
chemist’s office to 73.8% of patients and disposables,
when necessary, was supplied from hospitals to 53.8% of
patients. HEN was finished for 1,031 patients (34.1%)
during the period of study, 56.6% due to decease and
22.2% due to recovery of oral intake.
Conclusions: Data from NADYA-SENPE registry
must be explained cautiously because it is a non-compulsory
registry. In spite of the change in the methodology of
the registry in 2010, tendencies regarding HEN have been
maintained, other than oral routeObjetivos: Describir los resultados del registro de nutrición
enteral domiciliaria (NED) del grupo NADYASENPE
de los años 2011 y 12.
Material y métodos: Se recopilaron los datos introducidos
en el registro desde el 1 de enero de 2011 al 31 de diciembre
de 2012.
Resultados: Hubo 3021 pacientes en el registro durante
el periodo, procedentes de 29 hospitales, lo que da una
prevalencia de 65,39 casos por millón de habitantes.
97.95% fueron adultos, 51,4% varones. La edad media
fue 67,64 ± 19,1 años y la mediana 72 años para los adultos
y 7 meses para los niños. La duración media de la NED
fue 351 días y para el 97,5% fue el primer episodio con
NED. La mayoría de pacientes tenían NED por una enfermedad
neurológica (57,8%). La vía de acceso fue sonda
nasogástrica para el 43,5% y gastrostomía para el 33,5%.
La mayoría de pacientes tuvieron un nivel de actividad física
limitado y, respecto a la autonomía, 54,8% necesitaba
ayuda total. La fórmula de nutrición se suministró desde
las oficinas de farmacia para el 73,8% y los fungibles,
cuando fueron necesarios, desde los hospitales para el
53,8%. La NED se suspendió en 1.031 pacientes (34,1%)
durante el periodo de estudio, 56,6% debido a fallecimiento
y 22,2% debido a recuperación de la vía oral.
Conclusiones: Los datos del registro NADYA-SENPE
deben ser interpretados con precaución ya que se trata de
un registro voluntario. A pesar del cambio de metodología
del registro en 2010, las tendencias en NED se han mantenido,
salvo la importancia cuantitativa de la vía ora
Nutrición parenteral domiciliaria en España, 2019: informe del Grupo de Nutrición Artificial Domiciliaria y Ambulatoria NADYA
RESUMEN Objetivo: comunicar los datos de nutrición parenteral domiciliaria (NPD) obtenidos del registro del grupo NADYA-SENPE (www.nadyasenpe.com) del año 2019. Material y métodos: análisis descriptivo de los datos recogidos de pacientes adultos y pediátricos con NPD en el registro NADYA-SENPE desde el 1 de enero al 31 de diciembre de 2019. Resultados: se registraron 283 pacientes (51,9 %, mujeres), 31 niños y 252 adultos procedentes de 47 hospitales españoles, lo que representa una tasa de prevalencia de 6,01 pacientes/millón de habitantes/año 2019. El diagnóstico más frecuente en los adultos fue “oncológico paliativo” y “otros” (21,0 %). En los niños fue la enfermedad de Hirschsprung junto a la enterocolitis necrotizante, las alteraciones de la motilidad intestinal y la pseudoobstrucción intestinal crónica, con 4 casos cada uno (12,9 %). El primer motivo de indicación fue el síndrome del intestino corto tanto en los niños (51,6 %) como en los adultos (37,3 %). El tipo de catéter más utilizado fue el tunelizado tanto en los niños (75,9 %) como en los adultos (40,8 %). Finalizaron 68 episodios, todos en adultos: la causa más frecuente fue el fallecimiento (54,4 %). Pasaron a la vía oral el 38,2 %. Conclusiones: el número de centros y profesionales colaboradores con el registro NADYA va incrementándose. Se mantienen estables las principales indicaciones y los motivos de finalización de la NPD
Nutrición parenteral domiciliaria en España 2018. Informe del Grupo de Nutrición Artificial Domiciliaria y Ambulatoria NADYA
Aim: To communicate home parenteral nutrition (HPN) data obtained from the HPN registry of the NADYA-SENPE group (www.nadya-senpe. com) for the year 2018. Material and methods: Descriptive analysis of the data collected from adult and pediatric patients with HPN in the NADYA-SENPE group registry from January 1st, 2018 to December 31st, 2018. Results: There were 278 patients from 45 Spanish hospitals (54.7% women), 23 children and 255 adults, which represent a prevalence rate of 5.95 patients/million inhabitants/year 2018. The most frequent diagnosis in adults was “palliative cancer” (22.0%), followed by “others”. In children it was Hirschsprung’s disease together with necrotizing enterocolitis, with four cases (17.4%). The first indication was short bowel syndrome in both children (60.9%) and adults (35.7%). The most frequently used type of catheter was tunneled in both children (81.0%) and adults (41.1%). Ending 75 episodes, the most frequent cause was death (52.0%) and change to oral feeding (33.3%). Conclusions: The number of centers and collaborating professionals in the registry of patients receiving HPN remains stable, as well as the main indications and reasons for termination of HPN.Objetivo: comunicar los datos de nutrición parenteral domiciliaria
